The difference between sex and gender is very simple:

Sex = sexual biological identity

Gender = cultural construction

It is the individual who establishes their own identity. Identity is a work in progress, based on the model we ourselves determine. There is no inherent masculinity or femininity. We are not born as men or women; we are made into men or women. We play a role: that of masculine or feminine identity.

Masculinity is a relative concept; today's masculinity is different from that of ancient Greeks. Each social class also has its own sexuality. We can speak of "masculinities." Gender legacy is negotiated in society. There are cultures that do not accept people being "outside the box," with punishments and even death penalty, as in countries where homosexuality is not tolerated. Women in these countries do not have equal citizenship rights as men.

It is possible to subvert heterosexual determinism, despite the restrictions and cultural taboos of society. Today, it is possible to rewrite the body, imprinting one's own desires, emotions, and fears. A great example is Michael Jackson. Such a thing was unthinkable until 60 years ago. Today, the body is a dynamic entity. We defy the signs of age and even gender.

Psychosis often arises due to repressed identity when an individual spends too much time living in a way they do not like, subjecting themselves to discourses without building their own well-being. To build well-being, one often needs to oppose prevailing discourses in society. The more moral a society is, the more repressive it tends to be.

There are political and religious interests that construct gender identity as a fate; this allows legitimizing the tradition that places masculinity as strength, reason, etc., and femininity as sensitivity, intuition, etc. This legitimizes the status quo. (Well, fuck the status quo! :)
